Chapter 1

Ethan lay unconscious on the gurney, thirty-five years old, towering even in his motionless state, bruised, bleeding, and fighting for every breath. To the civilized world, he was a legitimate billionaire philanthropist, a titan of shipping, real estate, and hospitality in Chicago. To the underworld, he was the ruthless head of the Blackwood crime syndicate, a man whose name made hardened syndicate captains lower their gaze. To me, he was simply the man who left my favorite soup in the refrigerator just three hours earlier, texting me to drive safely through the storm.

Then the other door burst open.

A young woman in a drenched cream-colored dress stumbled in, her hands frantically cradling a prominent five-month baby bump. With two of Ethan’s most trusted bodyguards flanking her like protective gargoyles, she lunged toward the gurney, wailing for her husband, sobbing that her child could not grow up without a father. The nurses froze. The trauma team held their breath. One bodyguard finally met my eyes, his face draining of all color as he whispered, "Mrs. Blackwood..."

The pregnant stranger whipped her head toward me, her eyes wide with shock and sudden realization. "Mrs. Blackwood? Who are you?"

My throat burned, every ounce of professional poise screaming against the betrayal tearing through my chest. I looked right into her eyes, my voice steady despite the tremor in my soul. "I’m Claire Blackwood. Ethan’s legal wife."

The seconds stretched like wire pulled to its breaking point. Yet, beneath the shock, the betrayal, and the suffocating agony, my surgical instincts roared back to life. Ethan was coding. His intracranial pressure was spiking, and internal hemorrhage was threatening to shut down his organs. Whatever lies he had lived, whatever secret life he had built behind my back, I refused to let him die on my operating table before he looked me in the eye and confessed.

"Clear the trauma bay!" I roared, pushing past the pregnant stranger, my scalpel already gleaming under the harsh lights. "We are losing him! Prep for immediate emergency craniotomy and exploratory laparotomy right now!"

For the next four agonizing hours, I fought death itself for the soul of the man who had betrayed me. My hands moved with mechanical precision, suturing lacerations, relieving pressure on his brain, and tying off a ruptured spleen. Beside me, my surgical team watched in tense silence, acutely aware of the dramatic storm unfolding both outside the hospital windows and right inside our operating theater. Every sweep of my scalpel felt like carving away another piece of my own heart, remembering the anniversary dinners, the forehead kisses, the sweet domestic promises that were nothing more than a facade constructed by a master criminal.

When we finally wheeled him into the intensive care unit at 4:15 a.m., stable but deeply comatose, my scrubs were soaked in sweat and blood. Standing outside the glass doors in the dimly lit corridor were the pregnant stranger—whose name I would soon learn was Mia—and the two bodyguards. Mia looked exhausted, pale, and terrified, her hands still resting protectively over her swollen belly.

"Is he... is he going to make it?" Mia whispered, her voice cracking.

I pulled off my surgical cap, letting my hair fall around my shoulders, and stared at her with a cold, unyielding gaze that had stared down Chicago’s most dangerous mobsters. "He’s alive," I said coldly. "Because I am a surgeon who honors her oath. But when he wakes up, Mia—is it Mia?—you and I are going to have a very long conversation about exactly what you mean to my husband."

She flinched, stepping back against the wall, but before she could answer, a chilling voice echoed down the sterile hallway.

"Well, well, well. Look who’s playing guardian angel."

A tall, impeccably dressed man stepped out of the private elevator, surrounded by armed men in tailored black suits. It was Viktor Vance, Ethan’s primary rival and the most vicious enforcer in the North Side syndicate. A sinister smile curled across his lips as he looked between Mia, the ICU room, and me. The accident on the deserted highway hadn't been a random storm collision. It had been an assassination attempt. And with Ethan incapacitated, the war for control of Chicago’s underworld had just landed directly on my doorstep.
